Eligible Measures: Measures must meet Program energy-efficiency specifications. If you, or your contractor, are not sure of the specifications, please contact the Program before proceeding.

Eligibility: Program rebates are available for work performed on or after July 1, 2010. Rebates are not retroactive. All equipment must be new and have been purchased and installed prior to submitting a rebate application. Equipment must replace existing equipment to qualify for a rebate. Services or installation must be performed in homes built prior to 2000, homes built after 2000 do not qualify. Landlords and property managers are considered businesses under this program and are not eligible to receive a rebate. 

Rebate Availability: Rebates are available first come, first served, and according to date received, until Program funds run out. Rebate will not exceed purchase price. Rebate Applications must include all information requested. It is the responsibility of the applicant to submit a complete Rebate Application. Incomplete Applications will be returned to the applicant with a detailed explanation of why the Application was returned and how to reapply. You may resubmit a completed Application, but funds will not be held until an accepted form is processed. Additional terms and conditions may apply.

Rebate Amount: Rebates for energy-saving measures completed and/or installed as set forth in documentation accompanying this Application are limited to the amounts provided by the Program. Such amounts are subject to change. Contact the Program with any questions.

Final Assessment Rebate: For a limited time, the Program is offering a final assessment rebate to offset the costs of your Final Home Performance Assessment.  Funds are for a limited time only. Rebate values may vary.

Approval and Verification: Prior to any payment of incentives, homeowner's contractor will verify that the installed energy-saving measures meet Program specifications (as defined by Building Performance Institute and current Program requirements). Homeowner's home may also be selected for a quality control post-installation inspection. No warranty is implied by this inspection. State of Utah and/or their respective agents reserve the right to verify all services or work performed and to have reasonable access to the homeowner residence to inspect the work performed or installed under this program.

Application and Rebate Payment: This application must be filled out completely, truthfully and accurately. The homeowner must sign and submit the completed application along with a copy of all of the Required Documentation. Rebates will be processed on a first-come, first-serve basis and paid after: (i) completion and or installation of the energy-saving measures, (ii) verification of the completion and/or installation of the measures and (iii) timely submission of all required documentation of measures. Incentives will be paid directly to Homeowner. Please allow 4-6 weeks from receipt of completed application to process and mail rebate check. Please contact Utah Home Performance at 877-575-4177 to inquire about the status of a rebate application.

Proof of Purchase: The invoice documentation accompanying this application must itemize the products purchased and/or work performed. This proof of purchase must show a description of any installation or other labor charges, and be accompanied by proof of payment in full.   

Access and Evaluation: The Program and/or its representatives may request access to the property on which energy-saving measures have been completed and/or installed and may review and evaluate the project during and after completion. Homeowner agrees to provide reasonable access to the property for the purposes described herein.

Information Release: Homeowner agrees that the Program may include Homeowner's name, Utah Home Performance services and resulting energy savings in reports or other documentation submitted to the Utah State Energy Program.  Utah Home Performance will treat all other information gathered in evaluations as confidential and report it only in the aggregate.

Disclaimer / No Liability: Homeowner understands that, while the Program may provide rebate payments for work performed for Homeowner, the Program is not supervising work performed for Homeowner nor is the Program responsible in any way for proper completion of that work or proper performance of any products purchased. The Program is simply providing funding to assist Homeowners in implementing energy-saving measures. Homeowner assumes the risk of any loss or damage(s) that Homeowner may suffer in connection with installation of the measures. the Program does not guarantee any particular energy-saving results by its approval of this application or by any other of its actions.

No endorsement: The Program does not endorse any particular manufacturer, contractor or product in promoting Utah Home Performance. The fact that the names of particular manufacturers, contractors, products, or systems may appear on this application does not constitute an endorsement. Manufacturers, contractors, products or systems not mentioned are not implied to be unsuitable or defective in any way.

Property Rights: Homeowner represents that Homeowner has the right to complete and/or install the energy-saving measures on the property on which those measures are completed and/or installed.

Limitation of Liability: State of Utah and/or their respective agent's liability is limited to paying the rebate specified. The State of Utah and/or their respective agents are not liable for any consequential or incidental damages, or for any damages in tort connected with or resulting from participation in this Program. State of Utah and/or their respective agents do not warrant the performance of installed equipment expressly or implicitly. Rebate applications cannot be altered in any way.

Tax Liability: The Program is not responsible for any tax liability, which may be imposed on the Homeowner as a result of payment of any rebates or as a result of obtaining financing. The Program is not providing tax advice, and any communication by Utah Home Performance is not intended or written to be used, and cannot be used, for the purpose of avoiding penalties under the Internal Revenue Code.

Equipment Warranties: State of Utah and/or their respective agents DO NOT WARRANT THE PERFORMANCE OF INSTALLED EQUIPMENT, EXPRESSLY OR IMPLICITLY. State of Utah and/or their respective agents make no warranties or representations of any kind, whether statutory, expressed, or implied, including without limitations, warranties of merchantability or fitness for a particular purpose regarding the appliance or services provided by a retailer, manufacturer, vendor or contractor. Contact your retailer, manufacturer, vendor or contractor for details regarding equipment performance and warranties.

Homeowner's Certification: By signing this application, the owner certifies that he/she has performed all retrofit upgrades listed on this application at the defined location. Homeowner agrees that all information is true and that he/she has adhered to all program and equipment requirements listed. Homeowner understands that the rebate is through the American Recovery and Reinvestment Act of 2010 (ARRA) and is subject to special conditions governing ARRA funded projects.
